MAKING AN APPOINTMENT
REFERRING PROVIDERS
You may call 865.218-7470 to make an appointment for your patient.  If you would prefer simply complete our APPOINTMENT/REFERRAL form and fax it along with any pertinent medical records to 865.218-7471.  We will then contact you and the patient with the appointment information.

PATIENTS
To make an appointment call 865.218-7470.  Please be prepared to provide the following information:
  • Name
  • Date of birth
  • Phone number where you may be reached
  • Address
  • Social Security Number
  • Description of the Problem
  • Insurance Carrier
  • Insurance ID number
  • Name, Date of birth, and social security number of the insurance card holder



APPOINTMENTS
Patients with emergent problems can normally be seen the same day.  Urgent/Acute  problems can be seen within 24 - 48 hours. 
PATIENTS For your first appointment or initial consultation, please bring the following information with you:

  • Insurance Cards
  • Medication bottles or a current list
  • X-ray films related to your condition
  • Co-pay or Co-Insurance if required

To save time on your first first you can obtain and complete the New Patient Information forms before your appointment.
